Register new protocol as a File Type (rtsp://)

Is there a way to register the rtsp protocol with an application so I
can run rtsp://... urls directly from a command line?

I see the known protocols are registered under File Types but I don't
see any way to add a new one. New file types ask only for an
extension.

Yes, it's possible. This has to be done in the registry.
