Do you have a Fax Modem installed?
Do you show a Fax Printer in your printers and Fax applet in the control panel?
Do you have the document you want to fax stored as a file on the computer?

If so, connect a phone line to your fax modem.
Open the document and choose print from the "FILE" menu.
Change the printer to the fax printer.
Insert the Doctor's fax number when requested.
Choose "send"

If no to any of the above questions, take the document to the doctors office or to any of the local Fax Services like the UPS store or Kinko's.
