outlook reoccuring appointments

I have an intermittent problem with my new outlook running with Vista. every
now and then, for no reason the one off meeting I schedule is recorded with a
weekly reoccurance. I am not requesting this and have to go into the
meeting, the reoccurance section and turn it off (it usually takes 2 attempts
for the system to accept). Any ideas? is it a known fault?
