Remap and assign shortcuts

On the Microsoft help page it says: go to customize - select the tool you
want to give the shortcut to click modify- make sure the it has "&" the tool
name and then the shortcut key.

I can get all this to happen, but it doesn't do anything! If i hover over
the button it shows the added text. What am I missing?

Specifically, what I want to do is assign the pen and type selection tools
to buttons. The same button so it toggles would be the best. Could I make it
F2? How do I define ctrl, shift, or alt?

As a suggestion as well: being able to add and take away any functions from
the right click context menu would be awesome!

It seems like a little thing, but when you are taking notes it's really
annoying to have to move the mouse back and forth.
