Hey David, a couple of things.
1) Try running Outlook as an administrator before trying to enable the
OneNote addin. If you have Vista, the Outlook icon that appears at the top
of the start menu doesn't allow this but if you go into All Programs ->
Microsoft Office, then right click on Outlook and run as administrator. This
is the only way to enable certain addins in Outlook.
2) In the trust center, check the Disabled Items list. Make sure it's not on
there or else enabling the addin won't have any effect.
3) Try to do a Detect and Repair operation in OneNote.
