Word/Outlook mail merge to fax

We're having some problems using fax merging. I am following the guide at:

After reading the requirements for Fax Merging I installed:

Outlook FaxMail
Microsoft Fax
Personal Address Book

After adding the services to Outlook and rebooting the machine I start
up Word and try to do a merge the 'Faxes' selection in the 'Main
Document Type' dialogue is greyed out.

Is there a file I need to check, or template that needs installing?
