Our OneNote EMR Toolkit includes three different Diabetes Flow Sheets.

Send me a email and I'll send you a Trial copy. I'm not suggesting you
purchase the product, but if you run the Trial it will push a Diabetes Flow
Sheet to OneNote and you can use the template. (Hopefully you understand
what I'm proposing)

There are plenty of other Diabetes Flow Sheets on the web. (Google is your
friend)

It works with OneNote 2003 only.
